Get Healthy, Live Well — a comprehensive community health collaborative led by Tanner — has been working to provide community members in its service area the resources they need to lead healthier lifestyles. While public health focuses broadly on policies and prevention efforts that impact large populations, community health centers on improving outcomes within a specific region by addressing local needs and expanding access to healthcare. Another name for community health is population health, which emphasizes improving outcomes for groups of people within a defined area. With the help of its partners and volunteers, Get Healthy, Live Well is helping make communities a healthier place to live, learn, work, play and pray.

Get Healthy, Live Well is working to promote a healthy lifestyle and prevent chronic disease by:

  • Decreasing health disparities
  • Reducing obesity rates
  • Eliminating tobacco and vaping use
  • Increasing physical activity
  • Improving nutrition
  • Expanding mental health awareness

To help accomplish its mission, Get Healthy, Live Well hosts a variety of programs and launches multiple awareness campaigns that offer valuable resources, increase access to healthcare and strengthen community health across the region.

Get Healthy, Live Well Coalition

Get Healthy, Live Well is steered by a coalition of partners and volunteers who work together to establish, advance and maintain effective strategies that continuously improve community health and quality of life. Their efforts are guided by a comprehensive community health needs assessment and implementation strategy that identifies and addresses the needs of underserved communities, including the impact of social determinants of health such as housing, education and economic stability.

Community health workers play an important role in this effort by connecting residents to services, providing education, coordinating care, advocating for patient needs and supporting prevention initiatives such as mental health awareness and chronic disease management.

The coalition consists of businesses, educational institutions, community groups and faith-based organizations that have been working to provide community members the resources they need to lead healthier lifestyles. The coalition consists of several targeted committees:
